20 Years to 80 Years

Inclusion criteria

Inclusion criteria: 1. All type 2 diabetes patients aged 20 to 80 years; 2. living within the area heathcare provided by the community health centers assigned; 3. signed written informed consents; 4. can be followed-up 4years.

Exclusion criteria

Exclusion criteria: Patients with 1. Severe CVD,liver,kidney and other diseases; 2. end-stage cancers 3. migrant people; 4. not sign written informed consent; 5. any unstable diseases judged by investigators; 6. Medical conditions affect adhere to treatment; 7. diseases with limbs, skeletal, joints and muslces affect exercises; 8. Deep vein thrombosis.
