Inclusion criteria
Inclusion criteria: Inclusion criteria of intensive care units: A closed intensive care unit more than 6 beds in a tertiary hospitals that enrolled no less 100 mechanically ventilated patients per year was eligible. Inclusion criteria of patients: All adult patients requiring invasive ventilation for 24 hours or more were eligible.
Exclusion criteria
Exclusion criteria: 1. age less than 18 years; 2. transferred into the unit having been ventilated in another hospital; 3. dependence on mechanical ventilation for at least two weeks before enrollment; 4. GCS<7; 5. already enrolled in another research study.
