18 Years to 90 Years
Inclusion criteria
Inclusion criteria: 1) aged 18 years or above; 2) were diagnosed as chronic congestive heart failure,unstable angina,acute myocardial infarction or chronic obstructive pulmonary disease; 3) lived in the urban areas of Chengdu; 4) returned to home after discharge; 5) can contact by telephone or mobile after discharge; 6) did not participate in other studies; 7) agree to participate this study.
Exclusion criteria
Exclusion criteria: 1) patients with dementia; 2) patients with psychiatric disorders; 3) patients who can not speak and hear.
