Inclusion criteria

Inclusion criteria: New York Heart Association (NYHA) functional class II, III or IV

Exclusion criteria

Exclusion criteria: Uncontrolled arrhythmias, recent myocardial infarction, mechanical ventilation, cardiogenic shock, and volume depletion.
