Inclusion criteria
Inclusion criteria: (1) Aged older than 65 years; (2) Hypoxemic respiratory failure (PaO2/F iO2< 300 mmHg while receiving oxygen by high-flow (10 L/min=15min).
Exclusion criteria
Exclusion criteria: (1) Patients needed emergency tracheal intubation; (2) Patients enrolled more than 72h from the onset of disease; (3) Patients who have chronic lung diseases those have not been controlled; (4) Patients who have pleural effusion (=3cm by ultrasound), or complicated with other diseases ( by chest radiography); (5) Patients without civil capacity or with severe mental illness; (6) patients who were inappropriate use of NPPV (cardiopulmonary arrest, coma or unconsciousness, frequent vomiting or have aspiration risk, active upper gastrointestinal bleeding, facial deformity, massive respiratory secretions, hemodynamic instability, difficult to cooperate); (7) Patients can not be excluded with left heart failure; (8) Patients who can not provide informed consent or poor compliance patients.
