Inclusion criteria

Inclusion criteria: 1. Aged 18 years or more; 2. According to Chinese guidelines for the diagnosis and treatment of heart failure 2014 and grade II-IV on the New York Heart Association functional classification; 3. Left ventricular ejection(LVEF)=50% or LVEF>50% but NT-proBNP>400ng/l.

Exclusion criteria

Exclusion criteria: 1. Patients do not follow the follow-up plan; 2. Patients that are unable to communicate effectively.
