Inclusion criteria

Inclusion criteria: New admitted patients, 75 year or older, both genders, meet one of the following criteria: 1) the Barthel index score at admission decreased 20 points or more than baseline level; 2) the Barthel index score at admission is 30-80.

Exclusion criteria

Exclusion criteria: 1. patients who is already in the trial and is readmitted to the hospital; 2. Patient who should be in hospice care.
