Inclusion criteria
Inclusion criteria: 1. for hospitals: to choose two hospitals in every province: one is the academic hospital in provincial capital city, the other is the local hospital in the secondary city. The hospitals should meet the following requirements: to be the top of three hospitals in cities; have the majority of local patients; be general hospital; have ICU; some clinicians can join in this study; 2. for patients: in the observation period, new admitted patients and the patients who had already been in the ICU all came in (the age reach 18 years old).
Exclusion criteria
Exclusion criteria: 1. pregant woman or woman with positive pregnancy test; 2. brestfeeding woman who won't stop breast feeding; 3. patients known to allergic to sedative or analgesic agents
