Inclusion criteria

Inclusion criteria: For patient: 1. inpatient in Tianjin Metabolic Diseases Hospital; 2. Aged more than 50 years old; 3. HbA1c >=7%; 4. At lieast one family member lives in Tianjin; 5. Covered by Chinese Social Health Insurance for expenses of the health care and treatment; 6. Have the possibility to complete the follow up, include face to face once a month, phone 2 times per month, questionnaire survey 3 times per year; 7. Have the possibility to test the weight, blood pressure, blood lipid, kidney function, FBG, P2BG,HbA1c and 24 hours microalbuminuria; 8. Have the possibility to attend 2 courses, 90 minutes every time, 9:00 to 10:30 on Saturday morning. For the volunteer: 1. Family harmony; 2. Aged more than 25 years old; 3. Live in Tianjin; 4. spousal relationship or parent-child relationship; 5. live with the patient or often visit the patient; 6. Have the willing to help patient improve self management; 7. Have the possibility to help patient complete the follow up, include face to face once a month, phone 2 times per month, questionnaire survey 3 times per year; 8. Accept the comprehensive healthy education, family support skills and other courses of own free will; 9. Can take care of self and no severe disease; 10. Specific eligibilities to the adult children volunteers are: BMI >=25, FBG 5.6-6.9mmol/L or P2BG 7.8-11mmol/L, blood pressure 120-139/80-89mmHg; 11. Alternative eligibilities to the adult children volunteers are: abnormal TG; 12. the adult children volunteers have the possibility to test the weight, blood pressure, blood lipid, FBG, P2BG, HbA1c before and after the intervention.

Exclusion criteria

Exclusion criteria: Exclusion criteria of patients: 1) Severe debilitating disease. For instance heart failure, kidney failure, respiratory failure, and other diseases need to be hospitalized; 2) Rapid progress of diabetic complications; 3) learn barrier; 4) Cognitive deficit; 5) Communication barrier; 6) No abilities to carry out diet and exercise intervention. Exclusion criteria of the volunteer: 1) Severe debilitating disease. For instance heart failure, kidney failure, respiratory failure and so on; 2) learn barrier; 3) Cognitive deficit; 4) Communication barrier; 5) No abilities to carry out diet and exercise intervention.
