Inclusion criteria

Inclusion criteria: (1) Aged >= 40 years; (2) Patients at high cardiovascular risk defined by: (a) >=70 years of age; (b) 50-69 years of age with a history of diabetes mellitus and/or smoking or (c) confirmed CAD defined by a history of prior MI, PCI or coronary artery bypass grafting and/or >50% angiographic stenosis in at least one coronary artery.

Exclusion criteria

Exclusion criteria: (1) Patients are excluded if they are wheelchair bound provide informed consent; (2) Serious illness or condition (other than CAD life-expectancy to <24 months (e.g. malignancy).
