Inclusion criteria

Inclusion criteria: 1) presence of CVD, including coronary heart disease, hypertension, peripheral artery disease, rheumatic heart disease, or heart failure; 2) 18 years of age or older; 3) ability to read, write, and communicate; and 4) ability to communicate by phone

Exclusion criteria

Exclusion criteria: 1) unstable medical condition; 2) severe psychological disorders or mental health care; or 3) lack of provide written consent
