Inclusion criteria

Inclusion criteria: 1. aged 65 years or more; 2. MNA-SF less than 11; 3. sign an informed consent form;

Exclusion criteria

Exclusion criteria: 1. Patient has sever cognitive dysfunction and limited mobility; 2. Patient in an extremely frail state; 3. Patient is participating in other intervention project within 4 weeks;
