Inclusion criteria

Inclusion criteria: After the diagnosis and treatment by the three hospitals, the patient's condition is relatively stable, diagnosis is clear, the basic discharge standards to tertiary hospitals; patients = 60 years of age, ADL score of 60 points or less; patients have long-term care.

Exclusion criteria

Exclusion criteria: Patients who have undergone a medical treatment to determine the need for acute care or mid-term care; patients who refuse long-term care; who do not adhere to long-term care for more than six months; and who refuse to sign long-term care informed consent.
