Inclusion criteria
Inclusion criteria: For cross-sectional study 1. Aged =60 years; 2. lived in the local area for more and half years; 3. with one or more chronic disease or partial disability and dementia; In the intervention stage, beside above, also include Beijing registered residence or live in guangnei street for long time.
Exclusion criteria
Exclusion criteria: Elderly people refused to participant the project.
