intervention group:Individual health education, five sessions daily
intervention group:small group self management health education, one session daily
intervention group:Re-evaluation and instruction at the discharge
intervention group:telephone follow-up in one month after discharge, once a week, totally 4 times

Inclusion criteria
Inclusion criteria: 1. Aged 18 years old and more; 2. has been diagnosed acute stroke firstly; 3. suffered from slight to moderate neurological deficits with National Institutes of Health Stroke Scale score20; 6. To be able to communicate; 7. currently is Nanjing resident; 8. To be able to be reached by telephone; 9. To be able to sign informed consent.
Exclusion criteria
Exclusion criteria: 1. suffer from aphasia; 2. has severe or life-limiting co-existing disease; 3. has pre-morbid dependence; 4. Transferred to another unit during hospitalization; 5. is currently involving in any other research program.
