Inclusion criteria

Inclusion criteria: The patients getting diagnosed with type 2 diabetes and being managed by community physicians and aged 18 years or older can singe an informed consent with clear consciousness.

Exclusion criteria

Exclusion criteria: The patients getting diagnosed with tumor and serious physical dysfunction are excluded.
