Age
18 Years to 90 Years

Inclusion criteria

Inclusion criteria: 1. Type 2 diabetic patients; 2. sign agreements; 3. Without cognitive dysfunciton and with an intension to have regular follow-up; 4. Patients with life expectancy more than 2 years.

Exclusion criteria

Exclusion criteria: 1. With cognitive dysfunction; 2. Patients with reduced life expectancy less than 24-months due to recent diagnosis of advanced cancers and other life threatening conditions; 3. Patients who are unwilling to return for regular follow up.
