Inclusion criteria

Inclusion criteria: (1) diagnosis of first acute stroke; (2) =18 years old; (3) with slight to moderate neurological deficits (National Institutes of Health Stroke Scale score20; (6) can communicate; (7) Nanjing resident; (8) can be reached by telephone; (9) can sign an informed consent.

Exclusion criteria

Exclusion criteria: (1) aphasia; (2) length of hospitalization<10 days; (3) coexisting severe diseases (renal failure, heart failure, and end-stage diseases); (4) premorbid dependence; (5) transferred to another unit during hospitalization; (6) involved in other research programs.
