Inclusion criteria
Inclusion criteria: 1. Permanent residents aged 35 years old and above (born after December 31, 1979 and lived in project sites for more than half a year); 2. Voluntary participation and signed the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Newly identified patients with unstable angina, myocardial infarction, coronary intervention or coronary artery bypass grafting within half a year; 2. New stroke cases identified within half a year; 3. Severe dementia with difficulties to comply for community management.
