Inclusion criteria

Inclusion criteria: 1. Patients with established type 2 diabetes; 2. Patients aged 18-75 years old; 3. Patients who have been living in the community/village for more than 6 months and no plan of moving out within a year; 4. Patients who volunteer to participate in the study and have signed the Informed Consent Form.

Exclusion criteria

Exclusion criteria: 1. Patients with severe disease which leads to difficulties in late follow-up, or with any situation judged by investigators that might impact the follow-up visit; 2. Women in the process of, or plan for, pregnancy or breastfeeding 3. Patients who have been involved in any other clinical trials within 1 month.
