Inclusion criteria
Inclusion criteria: At the same time have the following five conditions before entering the study: 1. diagnosed with type 2 diabetes (WHO 1999 diagnostic criteria for diabetes), 12 months before enrollment HbA1c = 7.5%; 2. before the start of this experiment in June did not participate in other Lifestyle research and type 2 diabetes subjects; 3. Aged 50 to 75 years; 4. patients and caregivers living together; 5. patients with Beijing Medical Insurance Card, signed an informed consent before entering the group.
Exclusion criteria
Exclusion criteria: One of the following is not selected: 1. can not collaborators; 2. patients with severe heart, brain, liver, kidney, blood system and other complications and mental illness; 3. pregnancy or breast-feeding women; 4. limb dysfunction affect behavior intervention practitioners .
