Inclusion criteria
Inclusion criteria: decompensated chronic heart failure patients hospitalized in Cardiology department of West China Hospital
Exclusion criteria
Exclusion criteria: (1) unwilling to participate; (2) younger than 18 years old; (3) unable to read in Chinese; (4) did not possess a phone; (5) discharged to a long-term care facility; (6) dead before discharge; (7) planned to receive cardiac surgery within 6 months; (8) on waiting list for heart transplantation; (9) diagnosed with malignancy, or very ill that unlikely to survive to 6 months; (10) in severe psychiatric disorders; (11) participating other researches.
