Inclusion criteria
Inclusion criteria: 1. aged 40 to 70 years; 2. Computed tomography (CT) and digital substraction angiogram(DSA)-proven diagnosis of ruptured anterior circulation aneurysm; 3. admission within 72 h from ictus; 4. aSAH is graded as Fisher grade 3-4 on admission by initial CT scan; 5. Clincal symptoms categoried as Hunt-Hess grade 3-4; and 6. signed informed consent.
Exclusion criteria
Exclusion criteria: 1. SAH secondary to non-aneurysmal entities such as trauma, tumor, or arteriovenous malformation, cavernous malformation; 2. early death (defined as death within three-days of presentation); 3. preexisting dementia or modified Rankin score (mRS) =3; 4. prior hydrocephalus or ventriculoperitoneal shunt, endoscopic third ventriculostomy, or FLT; 5. coagulopathy; 6. known definite contraindication to operation; 7. The lamina terminalis is inaccessible during some anterior circulation aneurysm clipping such as in distal anterior/middle cerebral aneurysm; 8. DSA-negative SAH; 9. Patients complicated with vital organ dysfunction; 10. patient with poor compliance; 11. Pregnant 12. patients of extreme severity: with unstable vital signs, dilated pupils, Hunt-Hess grade 5
