Inclusion criteria

Inclusion criteria: Patient with mechanical ventilation for >or= 48 hr, Age >or= 18 years old, airway protected capabilities is normal and finished spontaneous breathing test: the patients had to have an improvement or resolution of the underlying cause of acute respiratory failure; adequate gas exchange as indicated by a ratio of the partial pressure of arterial oxygen (PaO2 ) to the fraction of inspired oxygen (FiO2 ) above 150~300 mmHg(1 mmHg=0.133 kPa) while breathing with an fraction of inspired oxygen (FIO2) or= 7.25, cough reflex is normal and no further need for vasoactive or sedative and analgesia agents(for example: dopamine<5ug/kg/min).

Exclusion criteria

Exclusion criteria: Age<18 years old, airway protected capabilities is poor and lost spontaneous breathing test
