Inclusion criteria

Inclusion criteria: Doctors, nurses and allied health staff of NTEC hospitals will be invited to participate in the survey study. For the interview study, interviewee are NTEC healthcare providers (doctors, nurses and allied health workers) who have experienced clinical incident. Participation is on voluntary basis.

Exclusion criteria

Exclusion criteria: Clinical staff who are not willing to participate
