Inclusion criteria

Inclusion criteria: From October 2012 to April 2013, all patients aged 65 years and above who were discharged from the geriatric wards of West China Hospital were sampled for study.

Exclusion criteria

Exclusion criteria: The patients were excluded if no medication was used or the medication administration records were unavailable or incomplete.
