Inclusion criteria
Inclusion criteria: 1. orally intubated (tube size>7.5 mm); 2. mechanically ventilated for more than 48 hrs; 3. evidence for reversal of the underlying cause for respiratory failure; 4. adequate oxygenation (PaO2/FIO2>200 mmHg; PEEP\8 cmH2O); 5. pH>=7.3; 6. hemodynamic stability (absence of active myocardial ischemia,absence of vasopressor use or\5mcg/kg/min of dopamine or dobutamine, heart rate\140); 7. absence of fever(T8or>10 g/dl in subjects withknown coronary disease; 9. Glasgow coma scale>10; 10. 18 years of age or older.
Exclusion criteria
Exclusion criteria: 1. known upper airway obstruction; 2. copious secretions with cough; 3. do notintubate/resuscitate status; 4. patients were not breathing spontaneously with adequate cough. Coughwas evaluated by placing a white card about 1.5 cm away from the end of the endotracheal tube andasking the patient to cough (3 to 4 times); 5. elder than <18 years old; 6. refused to sign the informed consent; 7. refused or can not be placed Edi catheter placement (such as: esophageal varices or obstruction, have esophagus surgery recently, facial surgery or upper gastrointestinal bleeding).
