Eligibility

Inclusion criteria

Inclusion criteria: 1. Neurocritical care patients 2. Intubated for neurological indications 3. Mechanically ventilated for more than 24 hours 4. Passed spontaneous breathing trial 5. Hemodynamically stable 6. Adequate gas exchange

Exclusion criteria

Exclusion criteria: 1. Pre-existing severe pulmonary disease 2. Tracheostomy in situ 3. Cervical spine or airway surgery affecting airway patency 4. Planned surgery within 72 hours 5. Hemodynamic instability or ongoing sedation/paralysis 6. Do-not-reintubate orders
