Inclusion criteria

Inclusion criteria: All consecutive patients of 1 month to 12years of age presenting with acute altered consciousness with less than one week onset admitted to EPR,PICU and paediatric wards will be enrolled.Altered sensorium is defined as decreased level of consciousness,attention and cognitive disorder

Exclusion criteria

Exclusion criteria: Excluded from study: Patient on high sedation and neuromuscular blocker. Patient with vision hearing speech or limb paralysis Patients with a prexisting neurological condition Parents not willing to give informed consent

MeasureTime frame
Primary outcome will be in-hospital mortalityTimepoint: At the time of discharge from hospital or death of the patient

Secondary

MeasureTime frame
Secondary outcomes will include neurological recovery at discharge, ventilation, and duration of hospital stay. Neurological outcome at discharge will be categorized as complete recovery, partial recovery, no recovery, or death.Timepoint: At the time of discharge or death of patient
