Inclusion criteria
Inclusion criteria: Adult patients admitted to medical wards and initiated on at least one systemic antimicrobial agent at any time during hospitalisation, with antimicrobial review at 48-72 hours after initiation
Exclusion criteria
Exclusion criteria: patients of the ICU, patients transferred from the ward to the ICU or from the ICU to the ward, patients who are not prescribed antimicrobials, patients discharged, expired or transferred within 48 hours of antimicrobial initiation, patients are not willing to give consent
