Inclusion criteria
Inclusion criteria: 1 Resolution or improvement of the condition requiring MV 2 Adequate oxygenation demonstrated by arterial oxygen saturation greater than 90 percent with an inspired oxygen fraction of 0.5 or less or a ratio of arterial oxygen partial pressure to FiO2 PaO2/FiO2 of at least 150 mmHg both with positive end-expiratory pressure or PEEP at or below 8 cmH2O 3 Absence of significant respiratory acidosis 4 Stable hemodynamic condition with systolic blood pressure between 90 and 160 mmHg without or with minimal inotropic support and heart rate below 120 beats per minute 5 Adequate level of consciousness without sedation 6 No excessive tracheobronchial secretions 7 Presence of an effective cough reflex
Exclusion criteria
Exclusion criteria: 1 Age less than 18 years 2 Pregnancy 3 Presence of thoracostomy pneumothorax or pneumomediastinum 4 Presence of flail chest or rib fractures 5 Pre existing cervical spinal injury history or diagnosis of neuromuscular disorders 6 History of diaphragm paralysis 7 The quality of the diaphragmatic ultrasonography images is poor and cannot be used for analysis 8 The patients next of kin refuses participation
