Eligibility
Inclusion criteria
Inclusion criteria: Patients - Adults aged more than 18 yrs, diagnosed with chronic or advanced life-limiting illness, able to provide informed consent or have a proxy respondent available when capacity is impaired. Caregivers - Primary informal caregivers more than 18 yrs, involved in patient care and decision making, willing and available for follow up assessment.
Exclusion criteria
Exclusion criteria: Patients - expected length of stay less than 48 hrs, severe cognitive impairment without available proxy respondent, unable or unwilling to participate Caregivers - Paid or temporary caregivers, unable to complete followup assessment.
