Inclusion criteria

Inclusion criteria: Active pain in mandibular molars with baseline score greater than 54mm on HP-VAS Prolonged response to cold test and EPT that lingers on after removal of stimuli Lack of Periapical Radiolucency on X-ray, except in cases of PDL widening.

Exclusion criteria

Exclusion criteria: Presence of large periapical radiolucency Periodontal disease other than acute apical periodontitis History of medication affecting pain perception Allergy to Local anesthetic agents bleeding disorders pregnancy and lactation suspected substance abuse

Anesthetic success rate determined as completion of access cavity preparation and initial root canal instumentation with Heft Parker Visual Analogue Scale score less than or equal to 54 mm without the need of supplemental anesthesiaTimepoint: 15 minutes after administration of local anesthetic during access cavity preparation and initial root canal instrumentation
