Eligibility

Inclusion criteria

Inclusion criteria: American Society of Anesthesiologist ASA physical status class I and II Both gender Age more than 60 years undergoing elective procedures. Patients or legally responsible relatives consent

Exclusion criteria

Exclusion criteria: ASA class III IV due to other systemic comorbidities and V Age less than 60 years Severe or uncompensated cardiovascular renal hepatic or endocrine disease Refusal to consent by patient or legally responsible relative. Patients who are intubated and on mechanical ventilation Emergency surgeries

MeasureTime frame
Evaluate the levels of neuroinflammatory markers MMP 9, NSE and systemic inflammatory markers IL 6 respectively pre and post anesthesia exposure in these patients Bilateral TCD parameters such as Systolic flow velocity, Diastolic flow velocity mean flowvelocity MFV pulsatility index of major arteries MCA ACA PCA and ICA are measured pre and postoperatively and compared for the new onset changesTimepoint: Evaluate the levels of neuroinflammatory markers MMP 9, NSE and systemic inflammatory markers IL 6 respectively pre and post anesthesia exposure in these patients Bilateral TCD parameters such as Systolic flow velocity, Diastolic flow velocity mean flowvelocity MFV pulsatility index of major arteries MCA ACA PCA and ICA are measured pre and postoperatively and compared for the new onset changes T0 baseline T1 immediate postop T2 12 hrs postop

Modified Rankin Scale at discharge and at 4 and 12 weeks after discharge Confusion assessment method CAM score in ICU Montreal cognitive assessment MOCA This will be used to assess baseline neurocognitive disorder and postoperative cognitive dysfunction between the 5th 7th postoperative day or at the time of discharge whichever is earlier Brief Screen for Cognitive Impairment BSCI telephonic instrument for postoperative neurocognitive disorder assessment at 4 weeks after dischargeTimepoint: 4 weeks 12 weeks
