Intervention1: Synchronous TR Protocol: Live video sessions (therapist-guided task-oriented training for upper limb reach/grasping and gait drills, 30-45 min/session). Real-time feedback/corrections u
progression from simple to complex tasks weekly. Control Intervention1: Asynchronous TR Protocol: Pre-recorded videos (mirror therapy for upper limb, PNF patterns for lower limb/gait, 30-45 min/day).

Eligibility

Inclusion criteria

Inclusion criteria: Adults aged 18-65 years( late youth to middle-aged adults and elderly ) Confirmed chronic neurological diagnosis Smartphone (Android or iOS) with camera, reliable internet (more than or equal to 5G). MMSE (Modified Mini-Mental State Examination) with a score more than or equal to 24 Caregiver available for setup/uploads. Written informed consent.

Exclusion criteria

Exclusion criteria: Acute or subacute phase (less than 3 months post-onset). Uncontrolled comorbidities (e.g., epilepsy, cardiac instability). Visual or hearing impairment uncorrectable to safe participation. Severe spasticity (Modified Ashworth Scale greater than or equal to 4). Insufficient home space or no caregiver support. Prior telerehabilitation exposure within 6 months
