Inclusion criteria
Inclusion criteria: Adults aged 18 60 years with chronic nonspecific low back pain persisting for at least three months and a pain intensity score of 3 or higher on the Numerical Pain Rating Scale will be eligible for inclusion. Participants must be able to understand instructions in either Hindi or English, be medically stable and capable of participating in supervised physiotherapy and exercise interventions, and be willing to provide written informed consent. Eligible participants should also demonstrate psychosocial and/or central sensitization-related features identified during screening and be available for all scheduled follow-up assessments.
Exclusion criteria
Exclusion criteria: Previous spinal surgery. Serious spinal pathology including fracture, malignancy, infection, or inflammatory spinal disease. Radiculopathy with progressive neurological deficits. Pregnancy. Severe cardiorespiratory disorders contraindicating exercise. Severe psychiatric illness limiting participation. Diagnosed neurological disorders affecting mobility or balance. Current participation in another structured rehabilitation programme. Cognitive impairment preventing understanding of instructions. Presence of red flag symptoms requiring urgent medical or surgical management.
