Inclusion criteria
Inclusion criteria: 1.Faculty members or practicing clinicians from the Departments of Pharmacy Practice, General Medicine, and Clinical Pharmacology with a minimum of five years of academic or clinical experience. 2.Registered clinical pharmacists actively engaged in hospital-based medication management or medication reconciliation services. 3.Medical practitioners (physicians) involved in patient care and familiar with the process of medication reconciliation. 4.Language and communication experts with prior experience in questionnaire development, translation, or validation to ensure linguistic clarity and cultural relevance. 5.Experts who are willing to provide informed consent and participate in the questionnaire validation process.
Exclusion criteria
Exclusion criteria: 1.Individuals from non-clinical or non-pharmacy backgrounds with no direct involvement in patient care or academic training related to medication use. 2.Experts who are unavailable or unwilling to participate in the validation process after initial invitation. 3.Participants with conflicts of interest that may bias the validation outcome.
