Inclusion criteria

Inclusion criteria: Adults aged 18 years and above Diagnosed with hypertension Registered at the selected HWCs

Exclusion criteria

Exclusion criteria: Individuals lacking basic proficiency in using an Android mobile phone will not be eligible to participate in the study Patients with severe cognitive impairment or unable to provide informed consent Patients with chronic debilitating diseases

To assess the hypertension care cascade in HWC catchment areas in Niuland. Capacity building of front-line workers in using a unique Android mHealth app for healthy lifestyle adoption and BP self-monitoring for hypertension management.
