Eligibility

Inclusion criteria

Inclusion criteria: Adults patients greater than 18 years, who are diagnosed with CKD based on KDIGO (stage 2 to 5), including dialysis patients. All comorbidity patients should be included.

Exclusion criteria

Exclusion criteria: Severe cognitive impairment or psychiatric disorders affecting the ability to participate in care. Pregnant or lactating women. Inability to adhere to treatment recommendations due to social, economic, or other barriers.

TO EVALUATE THE EFFECTIVENESS OF CLINICAL PHARMACIST INTERVENTION: 1.Drug-related problems will be assessed by using PCNE version 9.1 2. Patient health-related quality of life will be assessed by using FANLTC scale 3.
