Inclusion criteria
Inclusion criteria: Adults diagnosed with essential hypertension and listed in register of the two PHCs Those who have uncontrolled hypertension measured as per standard procedure Those who have poor hypertension related self-care practices measured by Hypertension Self Care Activity Level Effects (H-SCALE) People residing in study area for at least 1 year
Exclusion criteria
Exclusion criteria: I. People who are unwilling to give informed consent for the study. II. People who are critically ill and unable to respond. III. Pregnant and lactating mothers
