Eligibility

Inclusion criteria

Inclusion criteria: Newly diagnosed patients with Chronic Kidney Disease with GFR more than 15 and less than 60 ml per min per 1.73 meter square attending Nephrology unit Willing to participate in study Able to understand English Hindi and Punjabi Patients having android mobile phones

Exclusion criteria

Exclusion criteria: Patients on maintenance dialysis and post renal transplant patients
