Eligibility

Inclusion criteria

Inclusion criteria: Adults aged 18 years and above, ASA physical status I to III, non intubated patients, weight 40 to 80 kg, height 150 to 180 cm, scheduled for major surgeries with moderate to high expected blood loss greater than 500 mL. Eligible surgeries include major abdominal surgeries such as exploratory laparotomy and splenectomy, orthopedic surgeries such as hip fracture surgery, total hip or knee replacement and spine surgery, and gynecological or obstetric surgeries such as emergency cesarean section for placenta previa or accreta and hysterectomy for large fibroids or malignancy. Patients in whom preoperative type and screen is done as per institutional policy will be included.

Exclusion criteria

Exclusion criteria: Age less than 18 years, ASA physical status IV or higher, pre existing cardiovascular conditions affecting peripheral perfusion such as right ventricular failure or tricuspid regurgitation, peripheral vascular disease or limb injuries affecting PPI measurement, use of vasopressors before induction, preoperative red blood cell transfusion, intubation before baseline PPI measurement, incomplete intraoperative records, or loss to follow up.

Requirement of intraoperative red blood cell (RBC) transfusion, defined as administration of at least one unit of packed red blood cells during surgery and assessed throughout the intraoperative period.Timepoint: Time Points of Outcome Assessment Baseline: Before induction of anesthesia. Intraoperative: Throughout the surgical procedure. Postoperative: Within the first 24 hours after surgery.
