Inclusion criteria
Inclusion criteria: 1. Adult patients having pulpal necrosis in mature mandibular molar as confirmed by negative response to pulp sensibility test (cold and electric pulp test). 2. Radiographic evidence of apical periodontitis in the form of periapical radiolucency with periapical index (PAI) scores of 3 or more.
Exclusion criteria
Exclusion criteria: 1. Patients having a positive history of antibiotic use within the past month or requiring antibiotic premedication for dental treatment (including infective endocarditis or prosthetic joint prophylaxis). 2. Teeth having previous root fillings, unrestorable teeth, fractured or perforated roots, grade 3 mobility, periodontitis and history of recent periodontal therapy within the previous 6 months. 3. Teeth with established endodontic-periodontal lesions exhibiting less than 2 mm radiopaque bone between the root apex. 4. Patients with any known systemic inflammatory conditions or chronic diseases like diabetes, cardiovascular diseases, chronic kidney disease, and atherosclerosis.
