Eligibility

Inclusion criteria

Inclusion criteria: 1.Newly diagnosed patients of multiple myeloma 2.AKI as per KDIGO criteria requiring initiation of dialysis 3.Probable or definite myeloma cast nephropathy: Definite: Kidney biopsy showing cast nephropathy Probable: AKI with markedly elevated absolute serum FLC levels (involved sFLC more than 500 mg/L; more than 1000 mg/L considered highly suggestive), minimal albuminuria, and absence of alternative causes of AKI 4.Who give informed consent

Exclusion criteria

Exclusion criteria: 1.AKI primarily due to causes other than myeloma (e.g., sepsis related ATN, obstructive uropathy, glomerulonephritis) 2.Advanced chronic kidney disease (baseline eGFR less than 15 mL/min/1.73 m prior to myeloma diagnosis) 3.Prior long term dialysis dependence 4.Hemodynamic instability precluding intermittent hemodialysis 5.Refusal to provide consent 6.Patients who have received prolonged (more than 72 hours or more than 3 sessions) of conventional high-flux hemodialysis before initiation of MCO dialysis will be excluded.
