Inclusion criteria

Inclusion criteria: Diagnosed cases of Type 2 Diabetes Mellitus (ADA 2025 criteria). On stable anti-diabetic therapy for at least 3 months. Willing to consent and participate for 6-month follow-up

Change in glycemic control, measured as HbA1c (%) from baseline to 6 months, to assess the impact of structured Family Medicine Resident led lifestyle and medication counselling.Timepoint: Baseline After 3 months After 6 months
