Eligibility

Inclusion criteria

Inclusion criteria: 1. All patients undergoing anaesthesia for surgical procedures in the central operating theatre complex or designated remote anaesthesia locations at the study institution between 1 January 2025 and 31 December 2025. 2. Intraoperative events fulfilling the definition of a call-for-help, defined as any unanticipated clinical situation during the intraoperative period requiring escalation of care through active involvement of additional anaesthesia personnel beyond the primary anaesthesia team. 3. Events documented in the structured departmental call-for-help documentation system and verified through electronic medical records.

Exclusion criteria

Exclusion criteria: 1. Requests for assistance planned in advance for anticipated high-risk procedures. 2. Multiple activations occurring during a single continuous clinical episode (counted as a single event). 3. Events occurring outside the intraoperative period, including pre-induction and postoperative phases. 4. Cases with incomplete or unverifiable documentation.

Design outcomes

Primary

MeasureTime frame
To determine the distribution of intraoperative call-for-help events according to crisis type (airway/respiratory, bleeding/haemodynamic, and cardiac/pharmacological).Timepoint: At baseline.

Secondary

MeasureTime frame
1. Phase of anaesthesia at occurrence of call-for-help event (induction, intraoperative, emergence). 2. Type of event & contributing factors leading to call-for-help activation. 3. Response dynamics including response time, mode of communication, designation & number of responders. 4. Time to stabilisation & rate of intraoperative stabilisation. 5. Clinical outcomes including complication category, mortality, postoperative ICU requirement, & change in surgical plan. 6. Incidence of intraoperative call-for-help events per total anaesthetic procedures.Timepoint: At baseline, 1 week & at 4weeks.
