Inclusion criteria
Inclusion criteria: 1. Community-dwelling individuals aged 60 years and 6 months or above. 2. Non-bedridden or functional community-dwelling individuals, encompassing a broad spectrum of health statuses, including those with nutritional deficiencies, cognitive impairments, environmental risk factors, or pre-existing chronic conditions. 3. Residing in the study area for the past 7 months with plans to remain for the next 12 months, atleast. 4. Willing to provide written informed consent.
Exclusion criteria
Exclusion criteria: 1. Individuals who are completely bedridden or severely paralyzed. 2. Individuals with acute life-threatening medical or surgical emergencies at the time of recruitment. 3. Inability to provide minimal relevant details: Individuals (or their primary caregivers) who are unable or unwilling to complete the baseline screening questionnaire. 4. Severe cognitive impairment: individuals unable to understand the study requirements or follow the behaviour intervention instructions. 5. Multiple participants from the same household: To maximize the geographic capture area and minimize inter-group contamination, only one eligible individual per household (selected via lottery if multiple are eligible) will be included.
