Inclusion criteria
Inclusion criteria: Patients planned for open heart surgery New York Heart Association class one to three Stable vital signs with heart rate sixty to one hundred beats per minute, blood pressure within normal range, respiratory rate twelve to twenty breaths per minute, and oxygen saturation at least ninety four percent on room air Able to provide informed consent Diaphragmatic weakness with manual muscle testing grade two or more Left ventricular ejection fraction normal, mild, or moderate
Exclusion criteria
Exclusion criteria: Emergency cardiac surgery. Severe musculoskeletal disorders which hamper the exercise protocol. progressive worsening of exercise tolerance or dyspnea at rest or on exertion. unstable angina acute systemic illness or fever recent embolism less than 4 weeks. active pericarditis and myocarditis severe symptomatic aortic stenosis resting heart rate more than 120 bpm blood pressure drop 20 mmhg
