Inclusion criteria

Inclusion criteria: 1.Diagnosed with acute fissure in ano 2.Willing to provide informed consent

Exclusion criteria

Exclusion criteria: 1. Previous anal surgery 2. Known allergy or contraindication to diltiazem 3. Immunocompromised patient 4. Recurrent fissure 5. Fissure associated with tuberculosis 6. Fissure associated with haemorrhoids, fistula

Fissure Healing Rate will be better with local application of 2% of diltiazem gel Timepoint: Assessment will be done at 2, 4, and 6 weeks post-treatment.
