Eligibility

Inclusion criteria

Inclusion criteria: 1. Age: 20 to 40 years 2. Pain in back locates between last rib and inferior gluteal fold lasting more than 4 weeks subacute to chronic 3. Willing to participate. 4. Presence of active myofascial trigger points in superficial lumbopelvic muscles, specifically in one or more of the following muscles Erector spinae Gluteus medius Gluteus maximus Thoracolumbar paraspinal muscles 5. Both male and female. 6. NPRS Score: 4-6 Moderate pain 7. Oswestry disability index score: 21% - 60%. 8. Presence of at least one active myofascial trigger point in lumbar, paraspinal or gluteal musculature diagnosed by palpation reproducing the participant s familiar pain. 9. Reduced lumbopelvic stability - confirmed through Balance Master stability parameters e.g., decreased Limits of Stability scores, delayed reaction time, impaired weight-shifting ability.

Exclusion criteria

Exclusion criteria: 1. Patients having primary Pain area other than the lower back T12 to buttocks. 2. Leg pain as primary problem eg. Nerve root compression or disc prolapse with radiculopathy, central spinal stenosis. 3. Lumbar spine, lower limb or abdominal surgery and acute fracture in previous 6 months. 4. Pregnancy, inflammatory disease. 5. Scoliosis or any other spinal deformity. 6. Contraindications to ESWT active local infection at treatment site, coagulopathy or on anticoagulant therapy unless cleared by physician, malignancy at or near the treatment site, severe neuropathy. 7. Vestibular disorders, severe visual impairment or other conditions that independently impair balance e.g., recent lower limb injury within 6 months.
